Photo Release & Consent Form

I do hereby authorize AUT Merchandise Store, its agents, successors, assigns, subsidiaries and or affiliates to use all information supplied to said organization(s) by me in any or all of its publicity without limitation. Said picture(s), photograph(s) or information shall be and remain the property of AUT Merchandise Store. They may be used with or without using my name or using my initials of a fictitious name. Neither the copy nor the edited photographs have been submitted to me for inspection or approval prior to publication. I agree that I am donating this material and that my only compensation is as stated below on this consent form.

I agree that my only compensation is a free bottle of Gro-Aut Hair Oil every three months for one year from the first date of purchase of AUT Gro-Aut Hair Oil from AUT Merchandise Store for a total of 3 free bottles for the remainder of the year.

I agree that I must have made a purchase of at least one bottle of Gro-Aut Hair Oil from AUT Merchandise Store within the past 30 days in order to be eligible to receive the free Gro-Aut Hair Oil offer and that purchases made through AUT Merchandise Ebay Store does not qualify.

I agree to send AUT Merchandise Store a “before” and “after” photo within 2 weeks after 3 months of use of AUT Gro-Aut Hair Oil and every three months thereafter for 9 months.

I agree to only send photos in gif or jpg format to autmerchandise@comast.net.

I agree to only send photos that display a clear image. I understand that all photos sent not displaying a clear image may not be used. I also understand that sending photos without a clear image may cause my free oil shipments to terminate without notice.

I understand that I am eligible for free Gro-Aut Hair Oil shipments only after AUT Merchandise Store have received and approved my photo(s).

I understand that I will be notified by AUT Merchandise Store when my photo(s) have been approved.

I understand that I am only eligible for free Gro-Aut Hair Oil shipments after I have received an approval confirmation letter by an AUT Merchandise Store representative either via email or mail.

I understand that I will receive one free bottle of Gro-Aut Hair Oil after AUT Merchandise Store receives and approves my “before” and “after” photo(s) and I understand that I must send a current updated “after” photo(s) every three months thereafter for 9 months in order to continue my free Gro-Aut Hair Oil shipments for a total of three free bottles of Gro-Aut Hair Oil.

I understand that the material of AUT Merchandise Store is not meant to take the place of diagnosis and treatment by a qualified medical practitioner and that any application of the recommendations is at the user's sole risk.

I agree that I will avoid use of AUT Merchandise Store products if I have an allergy to any of the ingredients.

I hereby warrant and represent that I am at least 18 years of age.

I, the undersigned, do hereby consent to the above authorization and general release.
